About Us:
The Campbell River Hospice Society provides compassionate end-of-life and grief support to individuals of all ages through the support of trained volunteers and generous donors.
Position:
The Clinical Client Coordinator is responsible for managing and supporting the needs of all the CRHS’s clients and direct volunteers while maintaining a high-level quality of service. As the Clinical Client Coordinator, you will report to the Executive Director.
Qualifications & Experience:
Education and Experience:
Bachelor’s Degree in Social Work or an undergraduate degree; with a minimum of two year’s grief and loss counselling experience. Experience may be considered in combination with relevant education.
Membership in a Professional Association.
Demonstrated knowledge of the grief process.
Demonstrated knowledge of family systems.
Demonstrated experience with anticipatory grief, compounded grief, complicated grief and traumatic loss.
Knowledge and understanding of the philosophy of hospice palliative care and the needs of the dying, their families and the bereaved.
Knowledge & Abilities
Provide a high level of care and compassion.
Maintain a client-driven database.
Perform all Client Intakes and determine the need for services for the patient/family/caregiver.
Participate as a member of the Palliative Community Interdisciplinary Team and attend other meetings as appropriate.
Communicate with staff, clients, volunteers and community members effectively and professionally, including excellent written, verbal and computer communication skills (MS Office, Excel, PowerPoint, Access, etc.).
Work efficiently both independently and as part of a team in a fast-paced environment.
Organize, communicate and support all direct volunteer who is providing direct service to clients.
Provide debriefing to direct volunteers when requested.
Provide on-call support to volunteers on weekends and evenings when required.
Organize and coordinate clients and their needs with strong attention to detail.
Ensure accuracy, confidentiality and safekeeping of CRHS’s records.
Maintain communication with the bereaved family following the death of the individual hospice patient.
Attend Volunteer Education training.
Support Volunteer Coordinator in Volunteer Interviews.
Ability to maintain appropriate boundaries in the face of sorrow, pain and suffering.
Coordinates the delivery of services (individual, groups and ongoing follow-up) to ensure timely and appropriate services are provided to the client and family/caregivers.
Maintains an ongoing check-in process with all clients accessing service and volunteers providing direct service.
Ensure the quality of services through regular evaluation.
Ability to provide presentations on behalf of CRHS.
Willingness to adhere to the Mission and Values of the Campbell River Hospice Society.
Flexibility to perform other duties as assigned by the Executive Director.
